New Patient

As a new patient we want you to feel comfortable at your first visit. Here you will find frequently asked questions and answers.

How early should I arrive?

It is best to arrive 15 minutes early so the registration process can be thoroughly completed. Please allow plenty of time for travel so you may arrive on time. We do understand that things can come up, so please call to let us know if you will be a little late and we will do our best to keep you in the schedule. We want you to have as much time with your doctor as possible to answer all of your questions and being on time will ensure that happens.

Do I need to fill out forms before my scheduled appointment?

Please fill out the following 3 forms prior to your appointment. Having these forms completed before your visit will expedite your registration process, allowing you enough time with your doctor. These can be mailed to you upon request, or can be downloaded below:

What else should I bring?

Please bring your insurance card and a credit card with you to the clinic, and plan to make your payment at the time of the visit. We require all new patients to have a credit card on file or make a payment at the time of your visit. If you need a referral from your primary care doctor, please contact your doctor’s office to have this process completed prior to your appointment.

Do I need my medical records?

We do our best to assist you in gathering your medical information, but you know your medical history best. It is generally easiest for you to collect and bring a copy of relevant labs, pathology reports, x-ray reports, etc. to your visit. If you have had x-rays, ultrasounds, or scans of your thyroid, adrenal gland, or pituitary gland, please bring the images (films or disc) to your appointment.
