Our Philosophy
At Boulder Endocrinology, PLLC, our goal is to
provide superior care for people with hormone or
metabolic problems. We work in consultation with your
primary care doctor and other physicians involved in
your care to provide the best treatment available for
your endocrine problems. (Are you unsure what an

We are glad you are
considering an appointment with us, and we look forward
to seeing you in clinic!
Your First
Visit
There are several steps you can take to make your first
visit as pleasant and productive as possible:
1.
Arrive on time. Plan to arrive at least 15 minutes before your
scheduled appointment time, so that the registration
process can be completed appropriately. Make sure you
know where our clinic is located (see
Location),
and please allow plenty of time for travel so you may arrive
on time. (If you are late, we will do our best to see
you, but you likely will not have as much time with the
doctor).
2. Bring your insurance card with you to clinic, and
plan to make your payment or co-payment at the time of
the visit. If you need a referral from your primary care
doctor, please contact your doctor's office to have this
process completed prior to your appointment.
3. Complete
forms before appointment. Fill out
2 forms prior to your
appointment. These can be mailed to you upon request, or
can be downloaded below:

We must have these forms completed before you can see
the doctor. If you do not allow enough time to complete
registration forms, you likely will not have as much time to spend
with the doctor.
4. Gather
medical information.
We will assist you as much as possible in this process,
but our staff is unable to assess whether we have
complete information.
Incomplete information at the time of your appointment
leads to delays in evaluation and treatment. It is
generally easiest if you collect and bring a copy of
relevant labs, pathology reports, x-ray reports, etc. to
your visit. Relying on information to be faxed from your
doctor does not always work. If you have had x-rays,
ultrasounds, or scans of your thyroid, adrenal gland, or
pituitary gland, please bring the images (films or disc) to your
appointment.
If you need a signed
release form to give to your doctor or hospital to
obtain records, you may use this form if needed.
Release of Information
These simple steps
will make your first visit much more productive. We
appreciate your time and effort, and we look forward to
seeing you in clinic!
Consent for Treatment of
Minors
In the state of Colorado, treatment of patients younger
than age 18 requires parental consent. If you would like
your child to be able to be seen in clinic and receive
treatment when you are not present, please complete this
consent form.

Cancellation Policy
If you need to reschedule or cancel your appointment,
please notify us at least 48 hours prior to your
appointment. We request adequate time to be able to offer
your appointment slot to another patient.
